Our New Patient Forms are now available online! We are proud to make our patient forms available online so you can complete them at your leisure – saving you time waiting in our office and giving you time to carefully review our policies and procedures. Please complete the forms a minimum of 48 hours before arriving at our office. Click the icon below to download the form in PDF format. Submit your forms INDIVIDUALLY by e-mail by clicking on "Submit by e-mail" before closing each form and your forms will be presented for signature upon your arrival.
How to use our forms:

- Fill the form out on your computer using Adobe Acrobat Reader

- Email the forms to our office. (office@drtdredd.com) Submit each form individually before proceeding to the next form. Changes are not saved.
- Sign the forms when you arrive at our office.

Please allow 24 hours to receive a return call as each patient is very important and our goal is to address each patient's specific needs. Thank you.
Send Us Your X-rays
Have your x-rays forwarded electronically to our office. For offices that will comply with electronic transfer of radiographic records, request they be e-mailed to xray@drtdredd.com. Otherwise, please have records forwarded directly to the office by mail. |
